The purpose of this presentation is to describe implementation and evaluation of a palliative care consult team for providers and patients in an intensive care unit. Summa’s Palliative Care Consult Service (SPCCS) model offers interdisciplinary help to hospitalized patients, their families and staff upon referral of the attending physician. The focus of the model is mobilization of an interdisciplinary team, which includes a palliative care physician, an advanced practice nurse, a pharmacist, a chaplain and a social worker, all with expertise in palliative care. The team provides inpatient consultative services to health care providers within 24 hours of the initial request. Patients are assessed daily by a member of the team. Close collaboration with the attending physician and medical, nursing and social service staffs ensures coordination of care. Consultation services are intended to facilitate transition from curative care to palliative care, discussion of end-of-life issues, spiritual support, psychosocial support, discharge planning, coordination of home care and alleviation of distressing end-of-life symptoms. To date, the SPCCS has consulted for 255 patients.We estimate that 422 days in the ICU and 492 days on the general medical floor were saved due to SPCCS intervention. In addition, the service has received high satisfaction ratings from participating patients and families. The feasibility of using a case-control design to evaluate cost-effectiveness and improved patient outcomes from integration of the SPCCS into inpatient services at Summa Health System will be addressed.
